What Are All-on-X Dental Implants?
All-on-X is the evolution of the All-on-4 concept. The "X" stands for flexibility — rather than defaulting to exactly four implants for every patient, our surgeons customize the number of implants (typically 4, 6, or 8) based on your specific bone structure, density, and arch size. This individualized approach ensures the strongest possible foundation for your full-arch restoration.
For patients with denser bone, wider arches, or greater bite force, additional implants provide more support points and distribute load more evenly across the prosthesis. The result is a restoration engineered specifically for your anatomy — not a one-size-fits-all solution.
All-on-4 vs. All-on-X
If your bone density and jaw shape are well-suited to four implants, All-on-4 may be the right choice. When a more complex case calls for additional support — due to bone volume, arch width, or the need to replace an upper jaw — All-on-X allows Dr. Hoover to use 5, 6, or more implants to achieve the ideal outcome.
The number and position of implants is determined by 3D imaging and expert planning — tailored to your unique anatomy for optimal stability.
Additional implants distribute bite forces more evenly, reducing stress on any single implant and extending the long-term life of your restoration.
Like All-on-4, provisional teeth are placed the same day as surgery so you leave with a functional, beautiful smile from day one.
The Procedure
How the All-on-X Procedure Works
The process begins with a comprehensive consultation including a 3D cone beam CT scan. This imaging allows Dr. Hoover to precisely assess your bone volume and map out the exact number and angulation of implants needed for your case.
On surgery day, the procedure is performed under IV sedation or general anesthesia. Any remaining compromised teeth are extracted, implants are placed at planned positions, and a provisional full-arch prosthesis is secured immediately. You wake up with a complete set of teeth.
Over the next three to six months, your implants fuse with the jawbone through a natural process called osseointegration. Once healing is confirmed, your final custom prosthesis — crafted from high-strength, natural-looking materials — is placed, completing your restoration.
Candidacy
Who Is a Candidate for All-on-X?
All-on-X is an excellent option for patients with more complex full-arch restoration needs — those with larger arches, varying bone quality across the jaw, or cases where the standard four-implant configuration may not provide sufficient support. It is also commonly chosen for upper arch restorations, which often require more implants than the lower jaw.
Good candidates are generally in reasonable systemic health, non-smokers or willing to quit before surgery, and committed to the post-operative care protocol. A thorough evaluation at your consultation will determine whether All-on-X or another implant option is best for your situation.
This is determined by your 3D CT scan and evaluation. Most All-on-X cases involve 4–8 implants per arch. Dr. Hoover will recommend the number that provides the strongest, most reliable outcome for your specific anatomy.
In many cases, the strategic angulation of implants allows All-on-X to work without bone grafting. For patients with significant bone loss, a grafting procedure may be recommended before or during implant placement.
Most patients experience mild to moderate discomfort for the first several days, well managed with prescribed medication. You will follow a soft diet for the initial healing period. Most patients return to normal activities within a week.
